Heroin Mimicry: When Patients Don't Respond to Standard ED Opioid Overdose Management

Illicit opioid drugs of abuse have been diluted with many other deadly compounds that often mimic an experience similar to heroin. However, with all of the recent concern about fentanyl-laced products there is little discussed about these other chemicals.
During my lecture I plan on discussing how we got to this point as a society, what new street compounds should we be considering during the management of a suspect opioid overdose, and how ED management might differ when standard treatment is not successful. Several novel street drugs of discussion will be: Xylazine, ISO, carfentanil, etc... I will also review disposition concerns and safe discharge planning.
The lecture will be evidence-based and presented free of financial incentives.
Learning Objectives:
Identify chemical compounds of abuse that often mimic opioid intoxication yet do not respond to traditional ED management.
Escalate and adjust opioid overdose ED treatment protocols to mitigate patient harm and avoid intubation/CPR.
Understand our patients that struggle with addiction and how the ED can do a better job in regards to disposition/outpatient follow-up.
